Project Homeless Connect works directly with those seeking services, as well as with case managers and staff from other agencies, to make connections to hard-to-access resources, services and goods on a daily basis. These resources can help you make a successful transition from homelessness to housing. Whether you are a case manager referring a client or an individual in need of services, we will work with you to meet your resource or service needs. We serve the most vulnerable San Franciscans by delivering services in the office and via phone. Our intention is to streamline and close the gap between service and need.
Our Service Facilitators are ready to refer you to social, medical, and supportive services in San Francisco. We can help you find services including: vision, dental, general medical, mental health, addiction treatment and recovery services, harm reduction programs, self-help programs, food, clothing, computer access and classes, transportation, employment services, Medi-Cal, and other benefits.
PHC plans to continue providing services through Drop In Services, Problem Solving support, Off-Site Services, and Virtual Support; our Service Facilitators remain available to support Participants. For needs that cannot be provided virtually, especially financial needs, such as through our mail services or HandUp Point redemption, we will make appointments directly with Participants.

Resources
Hygiene Items & Basic Needs
We provide Simply the Basics basic hygiene items, basic needs, new socks, and other hygiene related needs upon request. Limited to availability.
Free Haircuts
We provide haircut vouchers to Womack Salon Academy. The salon provides a no-cost haircut to individuals with a voucher. * We will be updating this resource once we hear from Womack Salon Academy as they enter the next Phase for re-opening.
HandUp
HandUp helps donors give money to people in need. Homeless and other low-income neighbors have the opportunity to create an online profile and work with our Service Coordinators to set fundraising goals. Common goals include rental assistance, dentures, medical care, clothing, food, and other basic needs. Find out more about HandUp.* Currently Our Service Facilitators will be assisting participants with fulfilling their needs through HandUp redemptions.
Identification Support
Basic identification is needed to obtain housing, shelter, and employment. We provide no-fee vouchers to obtain a California Identification Card to eligible participants, assist in accessing birth certificates an passports.
Mail Services
For those without a secure address, utilizing our mailing services ensures receipt of income and housing applications for those without a secure home.
In-Kind Items
We receive donations of t-shirts, backpacks, sleeping bags, and other items. Items are limited and depend on donation. It is not a guarantee that we will have the item our participants mostly need in stock but please check with us regularly.All items are subject to availability and eligibility.
Problem Solving Support
We use tailored case advocacy and support to help people remove barriers and solve problems related to housing, employment, & health.
Resources and Referrals as needed
We work with Participants to understand their barriers, and support in connecting to additional care and services. Connections include access and appointments to Primary Care, Physical Health Specialists, Mental Health Care, substance abuse programs, employment services, and more.
Core Senses Program
We believe that it is hard to move forward with other goals if you are not able to address basic health needs. Our goal is to ensure that participants have easy access to quality, dignified and free health services. Here is what we offer:
